New Captain's License?
Here is what you need to do
The requirements are the same whether you are a member of Keys Consortium or not. To apply for a new Captain's license, you will need:
- Drug test within the past 6 months
- MRO signed 719P form
- MRO signed copy of your testing results
- Letter of membership from Keys Consortium

Complete the form below and we will receive your information via email and update your information in our database.
Complete USCG form 719P.
Get the form from your USCG packet or download it from the internet. Complete section 1 a bring it to the office when you test. (Find Form 719P)
Pay Invoice
You will receive an invoice from us via email that you can pay online. You can also pay your membership fee at the office when testing.
Complete your test
Come in to the office to test. 8:15am to 1pm for walk-ins or schedule an appointment after 1pm in our main office at 100430 Overseas Hwy Suite 300 Key Largo
Complete the form then come test
Cristel will be in the office of on Friday May 19th for Botox/Filler and other. Call to book your appointment
